New Patients

Your first visit to our office is important! We use it to establish a vital foundation towards our relationship with you.

During your first visit, we make sure to schedule enough time to get to know each other. To help make sure we can provide the best possible podiatric care for you and your loved ones, please take the time to look through this page and gather the information necessary for your first visit.

We value your time and wish to serve you efficiently by providing the information you need to prepare for your appointment. If you would like to learn more about our doctors – and even glean a couple of potential conversation topics to use during your first visit – you can read about Dr. Jeremy McVay and Dr. Candice Cooper on our About page!

Mission Statement

At the office of McVay Foot & Ankle, we will treat you like our own family and strive to exceed all of your expectations and needs.

Patient Forms

As part of your first appointment, we will gather important information and get to know you better. We understand that your time is valuable, and using it wisely will help us understand your concerns and questions in order to serve you best.

Background information, like your medical history, will be important in this process. For your convenience, you can download our new patient and records forms right from this page!

* You may need to have Adobe Acrobat Reader installed to print these forms. If you do not have it, you can click here to download it.

Preparing for Your Appointment

Being well-prepared for your appointment will ensure that your doctor has all of the necessary information needed to provide the best possible care for you. It will also help relieve any concerns or anxieties you might be feeling.

We have plenty of resources on our website that you can use both before and after your appointment. Learn about your symptoms by reviewing our conditions pages and our blogs on this website.

If you have any specific questions you wish to ask your doctor or specific items you feel are important that we know (such as unique symptoms, when they happen, etc.) we have always found it helpful to write them down and bring them with you. It’s easy to forget to bring certain matters up during an appointment, so having the written reminders can help. (And if you do forget something, don’t worry! You can always contact us afterward with any additional questions or notes.)

We look forward to meeting you on your first visit!

Insurance

Our practice is in-network with all major insurances. We do advise that you contact your insurance company to verify your coverage and benefits. We also recommend verifying whether a referral is required by your insurance company for your appointment.
